Sleeve Gatrectomy, one of the most common and highly successful procedure involves reducing the size of the stomach either using a gastric band of removing a part of the stomach to reduce the stomach size. Another procedure that is usually deployed is by resecting and re-routing the small intestine to a small stomach pouch, this procedure is called the gastric bypass surgery.
It has been proven by studies that these procedures lead to long term loss of weight. This loss in weight also helps recovery from Diabetes, improvement in cardiovascular situation. It’s found to have a reduction in mortality rate from 40% to 23% 1, however, the exact reason for this is not very clear.

Some of the mixed procedures are Gastric Bypass Surgery, Sleeve Gastrectomy with Duodenal Switch, Implantable Gastric Stimulation.
In this article, we talk about the most common type of procedure called the Sleeve Gastrectomy.

Step by Step Procedure
Sleeve Gastrectomy is a procedure in which the size of the stomach is reduced to almost half to 15% of its original size. This is done by surgical removal of large portion of the stomach along the curve of the stomach. The open edges of the stomach are then stitched together, this is generally done by surgical tapes or sutures or both depending on situation. This gives the stomach shape of a tube or more like a sleeve with banana shape. This procedure permanently reduces the size of the stomach, this reduction in size of the stomach is not reversible.

It’s a laproscopic procedure. The procedure is performed under general which keeps you asleep and pain-free. The surgery is usually done using a tiny camera that is placed in your belly. The camera is called a laparoscope. It allows surgeon to see inside your belly.

During the procedure

  • Surgeon makes 2 to 5 small cuts (incisions) in the belly, and the the scope and instruments needed to perform the surgery are inserted through these cuts.
  • The camera is connected to a video monitor in the operating room. This allows the surgeon to view inside the belly while doing the operation.
  • A harmless gas is pumped into the belly to expand the belly so that the surgeon has more room to work
  • Most of the part of the stomach is removed and the remaining portions of stomach are joined together using surgical staples. This creates a long vertical tube or banana-shaped stomach.
  • The surgery does not involve cutting or changing the sphincter muscles that allow food to enter or leave the stomach.
  • Once done, the scope and other tools are removed. The cuts are stitched closed.
  • The surgery usually takes about 60 to 90 minutes.

Weight-loss surgery might increase risk for gallstones and therefore expect your surgeon to recommend having a cholecystectomy. This is surgery to remove the gallbladder. It may be done before the weight-loss surgery or at the same time.
Sleeve Gastrectomy is one of the most accepted and fast growing weight loss procedure in Asia and North America. It’s a procedure almost as successful as the gastric bypass. This procedure is endorsed by the International Federation for the Surgery of Obesity and Metabolic Disorders and the American Society for Metabolic and Bariatric Surgery and has also been found to be safe and effective for children and adolescents, aged between 5 years and 21 years of age.

IVF – Choose the right center for best results

IVF (In Vitro Fertilization) is a very popular form of treatment for infertile couple. It has become so popular that there are many centers who operate without proper equipment and infrastructure in many small clinics and or newly self-proclaimed “fertility doctors” are delivering the IVF care. In such a situation, it’s important that you choose the right IVF facility for best results.

Let’s understand what IVF is
IVF is a common form of ART (Assisted Reproductive Technology). If the fallopian tubes in the female are damaged or the sperm is poor in male, this is an acceptable treatment.Other than that there are other situation of infertility also where this treatment works.
In this process, the eggs are fertilized in a laboratory, and the resulting embryos then are placed into the uterus 2 to 5 days later. This procedure achieves remarkable pregnancies even in women with fallopian tubes damaged to quite an extent, sterile husbands, and even “unexplained” infertility sometimes. If the husband’s sperm are weak, the eggs in most cases are fertilized using process called ICSI. This process reduces any risk of failed fertilization. Patients should always ask for IVF Pregnancy rate. A rate of over 50% per attempt is considered a good percentage.
Generally, Extra embryos are frozen and saved for a later date to reduce the cost of the next trial of pregnancy.

How Does this Work
Very simply put, the process is to fuse the egg and the sperm in a sterile environment outside female body and the embryo planted in female’s womb.

There are some preparation required in order to retrieve these multiple eggs for IVF. The woman must undergo injections with hormones and careful monitoring of her ovaries by ultrasound and her hormone levels by blood tests every day or every other day until she is ready for the egg retrieval.
For initiating the process, your doctor will understand your cycle and give an appropriate date for the procedure. On the specified day, eggs are retrieved by ultrasound guided needle aspiration under light sedation (in the operating room). This involves no surgical incision, and virtually no pain afterward. Only in case the ovaries are not accessible through transvaginal ultrasound, a laparoscopy procedure is conducted – a small incision is made near the navel and a slender laparoscope is inserted to guide the needle. The eggs are removed from the follicle through a needle connected to a suction device. Multiple eggs can be removed at one time. The mature eggs are then placed in a culture medium and are incubated. Healthy eggs are then fused with sperm to create embryos.

You just leave the hospital directly from the operating room, with little or no pain, and come back three days later to have the embryo (or embryos) placed very simply into the uterus through the cervix with a tiny catheter. No incision and no anesthetic are needed. An hour later you are able to go home. There is no pain from the procedure.

This whole process from egg preparation to retrieval and embryo placement can sometimes can be intimidating for the woman and the couple, but don’t worry as the centers we at Medi Buddy India suggest you will take very gentle care throughout and guide you gently through the whole process.

Post Fertilization
If you are pregnant, the IVF specialist will refer you to a obstetrician or other pregnancy specialist for prenatal care.
If you are still not pregnant, you will have to stop taking hormonal medicine and will get your period within a week. In absence of this, get in touch with your doctor. In case you want to go through another cycle of IVF, your doctor will suggest you the steps that needs to be taken for getting pregnant through IVF.

Some of the Factors that affect IVF results are
Age: Younger you are, better the chances of positive result using IVF. Women post 40 in some cases have to consider using donor eggs.
Reproductive History: Women who have given birth before are likely to have higher rate of pregnancy using IVF.
Lifestyle: Some of lifestyle habits has adverse effects on woman going through IVF. Smoking reduces the rate of success by 50%, Obesity also reduces the chance of a successful IVF. Excessive caffeine, alcohol also has negative effect on IVF results.
